Founder Insights: Rochester Physician Turns a Teddy Bear Into a Tool for Better Patient Care
ROCHESTER, N.Y. — What if a teddy bear could help patients manage their medications and improve their health?
For Dr. William Bayer, founder of Rochester-based HealthPozitive, that question has become the foundation of a health care technology company addressing one of medicine's most persistent challenges: medication adherence. Founded in 2019, HealthPozitive developed MediTeddi, an interactive teddy bear designed to help patients remember to take their medications without relying on smartphones, screens or complicated technology.
The product combines spoken reminders, simple controls and medication tracking to support patients who struggle to maintain consistent treatment routines. For Bayer, the idea emerged from decades of experience as a family physician and a patient encounter that changed how he thought about medication management.
From the Exam Room to Entrepreneurship
A Rochester native, Bayer attended Princeton University and Harvard Medical School before building a career in family medicine and medical education. Throughout his career, he repeatedly encountered patients struggling to follow prescribed medication schedules, even when they understood the importance of their treatment.
"As a family physician, I saw firsthand how difficult it can be for patients to take their medications consistently, and how limited the impact of simply reminding them could be," Bayer said.
One patient encounter became a turning point. A patient with poorly controlled diabetes left Bayer's practice frustrated by repeated conversations about medication adherence. Three months later, the patient returned in a wheelchair after suffering a massive stroke.
The experience stayed with me and inspired me to find a better way to help patients take their medications consistently, ultimately leading to the creation of MediTeddi," Bayer said.
Rather than developing another mobile application, Bayer began exploring a simpler, more accessible approach to helping patients establish consistent medication routines.
A Different Approach to Medication Management
MediTeddi is designed to provide medication reminders through an interactive stuffed animal, offering a companion-like experience without requiring patients to navigate digital applications. The device delivers spoken reminders once or twice daily, depending on the user's medication schedule.
Its controls are intentionally simple. Users can activate the device and establish a reminder schedule, press one paw to hear a greeting or hold both paws for five seconds to receive a spoken report on their medication adherence.
The approach is designed for older adults and patients who may experience challenges with digital literacy, cognitive function or complex medication routines. It also offers potential value for caregivers and health care providers seeking ways to support patients outside traditional clinical settings.
For HealthPozitive, the objective extends beyond reminding patients to take medication. The company is working to improve consistency and, ultimately, health outcomes.
Moving From Product Development to Clinical Evidence
One of HealthPozitive's most significant milestones has been the publication of peer-reviewed research examining MediTeddi's effectiveness. Bayer and researcher Anthony A. Sterns co-authored a study published in Experimental Gerontology titled "A Self-Determination Theory-Based Cognitive Prosthetic to Improve Medication Adherence in African American Older Adults with Diabetes."
The research examined the device's potential to improve medication adherence and clinical outcomes among older adults with poorly controlled diabetes.
"We have recently published a peer-reviewed journal article which documents the effectiveness of MediTeddi in significantly improving adherence, HbA1c values, and depression in older patients with poorly controlled diabetes," Bayer said.
HbA1c is a measure of average blood sugar levels over approximately three months and is commonly used to assess diabetes management. The pilot studies reported improvements in medication adherence, blood sugar control and depressive symptoms, although larger studies are needed to confirm the findings. <Cite ref="turn681385search1"/>
The findings represent an important step for HealthPozitive as it seeks to demonstrate the clinical value of its product to health care organizations, providers and potential customers. Broader adoption will depend on translating those findings into demand and establishing a sustainable commercial model.
The Next Challenge: Commercialization
Like many health care technology founders, Bayer has discovered that developing a product and demonstrating its effectiveness are only part of building a successful company. The next challenge is finding customers willing to invest in the solution.
"We have documented in a peer-reviewed journal the effectiveness of MediTeddi in improving medication adherence and clinical measures," Bayer said. "The next challenge is finding customers who value these improvements enough to buy our product."
Medication adherence presents a complex business challenge because the financial benefits of improved patient outcomes can extend across multiple stakeholders. Patients and families may benefit from better health, while insurers, health systems and other organizations may benefit from fewer complications and potentially lower treatment costs.
For HealthPozitive, identifying the right customers and demonstrating the economic value of improved adherence will be central to its next stage of growth. Bayer's experience reflects a challenge facing many early-stage medical technology companies: Clinical validation does not automatically translate into commercial adoption.
Why Rochester?
For Bayer, building HealthPozitive in Rochester means developing a health care solution in the community where he grew up. The region's established medical institutions, research capabilities and health care workforce provide a relevant environment for companies developing products aimed at improving patient outcomes.
Rochester also has a history of innovation in health care, manufacturing and technology, creating opportunities for entrepreneurs working at the intersection of those industries.
HealthPozitive represents a different type of regional startup: one built from decades of professional experience rather than a traditional technology accelerator or venture capital program. Bayer's path from physician to inventor demonstrates how problems identified through everyday clinical practice can become opportunities for entrepreneurship.
Beyond the Business
Outside of HealthPozitive, Bayer enjoys windsurfing, taking advantage of the region's access to lakes and outdoor recreation. His favorite local restaurant is Chuy's Taqueria, known for its Mexican cuisine, while Taproot Collective, a Rochester nonprofit supporting community gardens and youth employment, is among his favorite local organizations.
Although his career has included training at some of the country's leading academic institutions, Bayer remains connected to the community where his journey began.
As HealthPozitive moves toward broader commercialization, Bayer is focused on turning clinical evidence into meaningful adoption. His experience offers an important lesson for Rochester's startup community: Innovation does not always begin with a new technology. Sometimes it begins with recognizing a familiar problem and finding a different way to solve it.
For Bayer, that problem is medication adherence. And his solution comes in the form of a teddy bear.
